We accept referrals from the person/client themselves or you may submit a referral on someone’s behalf (e.g. as a family member/informal carer or professional/service) if you have their consent.

Once you have submitted the referral form, we will be in contact within 3 days. If you’ve completed the form for yourself, we will email you:

  • Our information pack

  • An intake form

  • A Service Agreement (once you’ve completed the intake form)

  • A link to our online booking system with the option to schedule a free initial chat
